OCI documents and U Visa stickers in favor of applicants (who applied for OCI status in 2004 & 2005) listed below have been received in the Consulate. Accordingly, applicants whose names etc appear in the list are requested to collect their OCI documents either by mail or in person. To receive the OCI documents by mail, applicants may forward their US/foreign passports, and a copy of acknowledgement slip received from the Consulate, to: OCI Cell, Consulate General of India, 455 City Front Plaza Drive, Suite # 850, Chicago, Illinois 60611. Envelope containing such documents should clearly be marked "FOR OCI DOCUMENTS 2004-05' in bold letters. Applicants still in possession of valid Indian Passports/PIO Cards, may ensure to include these as well for cancellation and return to the respective applicants. Failure to include valid Indian passports/PIO cards may cause delay in dispatch/release of OCI documents. OCI documents can be collected in person on any working day strictly between 4 to 5 pm. Collection of documents on behalf of others, including members of family, except minors, will be subject to submission of a letter of authority to that effect from the applicant. The applications submitted in 2004-05 were not assigned any File Number or Reference Number. Thus for the convenience of applicants, the names of the applicants, in favor of whom the OCI documents have been received, have been posted on the web. Those who applied afresh, in response to a recent announcement for 2004-05 applicants, may kindly look for their names/file number towards the end of the list indicated in Blue Color and take further steps as indicated in para l above or in 2006-2007 list. The list is updated each time additional documents are received.
