A. SALIENT FEATURES
This
service can be availed for re-issuance/issuance of duplicate OCI
documents in the following category of cases:
Ø
In case of issuance of new US/foreign passport upon expiry of
validity of existing passport.
Ø
In case of change of personal particulars viz. nationality, name,
father’s name, date of birth etc.,
Ø
In case of loss/damage of OCI registration certificate/U visa.
Ø
In case of change of address/occupation
1. In
case of issuance of new foreign passport:
OCI registration certificate and ‘U’ visa sticker contain the
passport number of the OCI holder. Accordingly, each time a new
passport is issued to an OCI holder; it has to be captured on the
registration certificate and U visa sticker. This would require
re-issuance of OCI registration certificate and U visa sticker each
time a new passport is acquired by the applicant in the
following cases
Ø
Up to the completion of 20 years of age, OCI documents have to be
re-issued each time a new passport is issued to the holder.
Ø
Between 21-50 years of age,
there is no need to seek re-issuance
of OCI documents each time a new passport is issued. However, if
the applicant desires, he/she may avail the service for re-issuance
of OCI documents.
Ø
After the completion of 50 years of age, OCI documents have to be
re-issued once after issuance of a new passport
2. In
case of change of personal particulars viz. name, date/place of
birth, father’s/mother/s name etc.
In
case of change of personal particulars (name, father’s name or date
of birth) while submitting online application, OCI holders may seek
re-issuance of OCI documents by availing this services. They have
to enclose the documentary evidence for the change in personal
particulars requested. In case of change of photograph, personal
interview with the Competent Authority will be required before the
grant of the service.

In case of Loss/damage of OCI
registration certificate/visa.
In case of loss of OCI documents,
a copy of the complaint filed with the Police authorities (where the
document is lost) shall be enclosed. Personal interview of the
applicant shall be taken by the Competent Authority before the grant
of the service.

In case of change of address/occupation
In case
of change of address/occupation, new set of OCI documents will not
be issued. The applicant may take a print out of the new
address/occupation after online submission and keep for record
purposes. No fee shall be charged from the applicant.
B.
PROCEDURE OF APPLICATION
Before filling the online application, Guidelines and
Instructions for filling application form explained below may be
perused so that there is no mistake in submission of the
application. Further, the details regarding Fee and
Offices where applications have also been explained hereher which
may also be perused. The processing time would be around 10 to
12 weeks from the date of receipt of application in the Consulate.
Before the expiration of the said period, it will not be possible to
address status queries in this regard.
In
case of any emergency,
applicant can undertake travel to India by carrying both, old and
new US/foreign passports, alongwith OCI document (booklet).

point ‘D’ in the information below. Applicants are once again
advised to read following carefully before accessing application
on-line.
(1)
After accessing the OCI Miscellaneous Services application on-line,
having gone through the Guidelines and Instructions for
filing the application form, the applicant has to fill in at
least one of the following data fields in the application:

U-Visa number
-
OCI Registration number
-
OCI File number
(2)
Thereafter, the applicant has to fill in all the following
fields:
a.
Current Passport number
b.
Date of birth
c.
Place of birth
d.
Mother’s name
The applicant would be able to proceed ahead and seek the particular
service(s) only if, the above data (except 2 (i) above) matches with
the OCI database.
This feature has been incorporated as a security measure so that the
scheme is not misused.
(3)
The applicant thereafter has to select any one or more of
the following services:
Ø
Change of Passport particulars i.e. No., date/place of issue etc
Ø
Change of Personal particulars i.e. name, date/place of birth etc
Ø
Loss/damage of OCI certificate/visa
Ø
Change of Address/Occupation
(4)
After selection of the service(s) to be availed, a Registration Form
will appear having only the relevant fields, which need to be
entered as per the services (s) sought. The applicant has to fill
in all the relevant fields afresh online. The reasons for
availing the services shall also be filled by the applicant.
(5)
Upon submission of the Registration Form on-line, a hard copy of the
application indicating Reference Number shall be automatically
generated by the computer. The applicant has to sign the
application form, paste the photograph and append his signature in
the box provided and submit two independent sets of
the application form along with the relevant enclosures (with both
copies of application) and fee as mentioned in the instructions.
The application can be submitted to the Indian Mission/Post/Office
in whose jurisdiction the applicant is ordinarily residing
irrespective of the fact whether the same Indian Mission/Post/Office
has issued the original OCI documents or not.
Each applicant shall submit separate application form.
No hard
copy of the application shall be generated in case of change of
address/occupation since no application needs to be submitted to the
Indian Mission/Post/Office.
(C)
Documents to be submitted alongwith application
:
(a)
In case of issuance of new foreign passport
:
i)
Two copies of printouts of the online application. The applicant
must affix one colour photo [35 mm x 35 mm] on each form and sign
in the signature box
ii)
ii)
Copies of existing OCI registration certificate and U visa pasted
on existing foreign passport.
iii)
Two copies of new passport
iv)
Fees through Bank Draft/Money Order.
(b)
In case of loss of OCI Registration Certificate and/or OCI Visa :
(i)
Two copies of printouts of the online application. The
applicant must affix one colour photo [35 mm x 35 mm] on each
form and sign in the signature box.
(ii)
Copy of the complaint lodged with Police authorities
(iii)
A note giving details of the circumstances of loss.
(iv)
Copies of existing OCI registration certificate and U visa pasted on
existing foreign passport of (In case a copy of the registration
certificate and/or visa was not taken prior to the loss, these may
not be enclosed.)
(v)
Fees by way of Money Order or Postal Order or Cashier Check valid
for atleast 1 year.
(c)
In case of change of personal particulars
(due to change by the applicant or due to wrong filling of
particulars by the applicant in online application)
(i)
Two copies of printouts of the online application. The
applicant must affix one colour photo [35 mm x 35 mm] on each
form and sign in the signature box.
(ii)
Copies of existing OCI registration certificate and U visa
pasted on existing foreign passport.
(iii)
Two copies of new passport
(iv)
Copies of the document (s) in support of the change requested {for
example new Citizenship certificate in case of change of
nationality}
(v)
Fees through Bank Draft/Money Order
(d)
In
respect of change of address/occupation -
no hard copy of
the application shall be submitted to the Consulate.
Accordingly, the issue of Payment of fee does not arise.
(D)
Fee & Mode of Payment:
(a)
US$25 for re-issuance of OCI
documents because of acquisition of
new US/foreign passport or change of
name or date/place of birth etc
(b)
US$ 100 for issuance of duplicate OCI
documents in case of
loss/damage of old OCI /Foreign passport carrying OCI Visa sticker.
NOTE:
(1)
In addition to the processing fee (for
each applicant)
indicated above, please
ensure to include Mailing Fee
of $20/- for first applicant and $1/- each for additional applicants
if applying under Family Group.
(2) Processing fee and mailing fee can be paid through
one Cashier Check/Money Order/Postal order drawn in favour of
Consulate General of India, Chicago.

Both OCI
registration certificate (booklet) and U Visa sticker shall be
issued afresh in respect of 6(a) to 6(c) above.
No fee shall be refunded if the service(s)
requested is not granted. Processing time taken will be
about 10 to 12 weeks from the date of receipt of application.
No individual intimations are sent. Applicants may check the status
of their applications only after expiration of the above period by
accessing the “On-line Status Enquiry”.
Incomplete applications will be sent back at applicant’s cost and
Consulate will not be responsible for loss of documents during
transit in mail.
No priority will be extended to such application(s) upon its/their
re-receipt in the Consulate and will thus be treated in the same
manner as all other applications.
(F)
Where
to apply
Applications complete in all respects and arranged in the order
stated above, alongwith prescribed fee should be mailed to
the following address by applicants living within the Consular
jurisdiction of CGI Chicago only:

Incomplete applications, on any count, will be sent back unprocessed
to the applicant at his/her cost. All applications will be
processed in the order of their receipt in the Consulate
irrespective of the category of mail. Kindly refrain from make any
query before the expiration of 10 to 12 weeks after the date of
application having reached the Consulate.
