GUIDELINES  FOR  PASSPORT  &  CONSULAR  SERVICES

 

A.  New Passport : (Alphabet ‘F’ appearing after the date of expiry indicates that the validity cannot be extended further.  Thus the applicant has to apply for a new passport. This service can be sought within one year before the date of expiry or within 6 months after the date of expiry without late fee.  Normally 5 years valid passport is issued in the case of minor and 10 years valid passport is issued in the case of an adult. In case of passport where the validity expired more than 6 months ago, kindly see the information under appropriate head below.)

 

1.      Application form (see 8 below) for New Passport, duly filled in and signed by the applicant (no column left blank).  In case of minor or those who cannot sign, Thumb Impression (Right Hand Thumb Impression for female and LHTI for male).

2.   Photographs: 4 recent colour photographs of 3.5 cm x 3.5 cms against white background showing full frontal view of full face. Out of these one photograph is to be pasted (do not staple this photo) on the first page of the application in the space provided for the purpose. Photographs with with other than white background will not be accepted.

2.      Existing passport  & copies of first three and last two pages (notarization not required if submitting application in person).

3.      Proof of residence:  any two of  i.e. copy of utility bill or Dr Licence or Lease Deed etc

4.      Visa Status:  copy of Pmt Resident Card(both sides) or Employment authorization card or copy of page showing  HI/B1 etc visa – in case of apply by mail, pl get this notarized.

5.      Passport Fee:  (i) Adult: $ 40/-  (ii) Minor (under 15 yrs): $ 25/- by way of Money Order/Postal Order/Cashier Check drawn in favour of Consulate General of India, Chicago

6.      Mailing Fee: $ 20/- for Express Mail or $10/- for priority mail.   Consulate will not bear any responsibility for loss of documents during transit in mail.

7.      PROCESSING TIME:

              Submitted at counter :  5 to 7  working days

              Applied by mail : 15 to 20 working days + mailing time

NOTEPassport to minor (under 15 yrs of age) are issued for a validity of 5 years only.  Emergency services are not rendered in r/o documents received by mail.  Incomplete applications are sent back at applicants cost without prior notice.  Consulate does not bear any responsibility of loss during transit in mail. 

 

B.   Extn  of Short Validity Passport   -   (apply for this service if the current passport  has been issued for less than 10 years validity under Tatkaal Scheme or for any other reason.  This is not applicable in case of minors as only 5 year valid passport is issued to them.  As such minors have to apply for a new passport after 5 years.  Application for new passport is same for minors and adults.

 

1.       Misc Services form (see 10 below), duly filled in and signed (no column left blank)

2.       Current passport  &  copy of first three and last two pages (plus any page containing an important observation).

3.       Photographs: Two recent coloured photographs against white background only – one to be pasted (not stapled) on the form and the other to be stapled.  Photographs with other than white background will not be accepted.

4.       Visa Status: copy of Pmt Resident Card(both sides) or Employment authorization or page containing valid visa – if applying by mail pl get this notarized

5.       Proof of residence” either of the two proofs i.e. copy of any utility bill or Dr Licence or Lease Deed or State ID  etc.

6.       Fee: $ 20/-

7.       Reference fee: $ 20/- applicable only if current passport was not issued from Chicago.

8.        Mailing Fee: Express Mail: $ 20/-  Priority mail: $10/-   OR adequately stamped self addressed envelope for return of documents.  No tracking numbers are furnished for Priority Mail and SASE.

9.       Processing Time: 10 to 15 working days.  However in cases where prior approval is required, it may take 30 to 45 days.

C.   Lost/Damaged Passport  (service not available by mail)

 

1.       Lost/Damaged Passport application form (see 14 below) - duly filled in and signed (no column should be left blank)

2.       Photo copy of Lost Passport - first 3 and last 2 pages or complete details of lost passport i.e. number, date & place of issue, period of validity etc.

3.       Visa Status: Copy of Pmt Resident Card (both sides) or Employment authorization card or HI or H4 or L1 or L2  etc visa

4.       Photographs: 6  recent colored photographs of  3.5 cm x 3.5 cms against white background with frontal view showing full face. Photographs with other than white background will not be accepted.

5.       Two proofs of residence i.e. copy of utility bill or Dr License or Lease Deed or bank document etc

6.       Affidavit – download along with form (see No.14 below) on the website also available at the Reception Desk. 

7.       Personal Particulars form, available at website along with form (4 copies) duly filled in – can be down loaded alongwith application form (see No.14 below) ,  also available at Reception – paste one photo on each copy of Personal particulars form.

8.       Incident Report: giving complete details about how, when and where the passport was lost duly signed by the applicant.

9.       Police Report in original, giving complete details of the loss reported.

10.   Fee: $ 125/-

11.   Referral fee: $ 20/- (if passport reported lost/damaged was not issued from Chicago).

12.   Affidavit fee:  $10/- for notarization of Affidavit by the Consulate

13.   Processing  time: same as for New Passport (if lost passport was issued from Chicago).  In cases where reference is required to be made to the Passport Issuing Authority which issued the passport reported lost, it may take up to 45 days.

NOTE:   Applicant need to present the application in person, with all supporting documents.  Issuance of duplicate passport is subject to receipt of clearance from concerned offices if the lost passport was not issued from Chicago.

 

D.   EXPIRED PASSPORT  (6 months or more) – service not available by mail

 

1.       New Passport application form(see 12 below), duly filled in and signed

2.       Existing passport and photocopy of the passport (first three and last three pages)

3.       Visa : Copy of Pmt Resident Card(both sides) or Employment authorization card or HI/B1 etc visa

4.       Photograph: 4 recent coloured photographs of 3.5cm x 3.5 cm  with frontal view showing full face against light background.

5.       Affidavit – available on website alongwith form (see 12 below), also available at Reception Desk.

6.       Two Proof of residence i.e. copy of utility bill or Dr Licence or Lease Deed or state ID etc

7.       Fee: $ 40/- for adult and $ 25/- for minor (under 15 yrs)

8.       Late Fee: $ 15/- (adult & minor) – if the validity of the passport expired more than 6 months ago.

9.       Mailing charges: Express Mail: $ 20/-; Priority mail: $10/- Reference Fee: $ 20/- (if  existing passport not issued from Chicago).

10.   Processing Time: 30 days for prior approval cat.plus transit time in mail both ways.  In some cases where addl information is required it may take some more time.

NOTE: In case of existing passport’s validity expired more than 4 years ago, issuance of new passport is subject to clearance from concerned authorities in India and the processing time will vary accordingly. No status queries will be entertained.

 

E.   Power of Attorney    (this service is not rendered by mail)

 

1.      Miscellaneous services form (see 10 below), duly filled in and signed, no column be left blank.

2.      Power of Attorney document (in duplicate i.e. two copies) with a photograph of the Executant duly affixed on each copy, near his/her signature at the end of the document.

3.      Photocopy of  passport, first two and last two pages only (first page to be notarized if the executant(s) is/are holding US passport). PoA document(s) is/are attested by the Consulate only if the same are to be produced in INDIA only.

4.      Visa: Copy of Pmt Resident Card(both sides) or Employment authorization card or HI or B1/B2 or L1/L2 visa etc – not applicable if the executant is holding US passport.

5.      Residence Proof: Two proofs of residence i.e. copy of utility bill or Dr Licence or Lease Deed etc – not applicable in case of company documents.  This is required just to ensure that the applicant is residing within the Consular jurisdiction of the Consulate.

6.      Passport in original to be presented, if asked the Consular Officer – not applicable in commercial documents.

7.      Executant(s) shall affix his/he/their signature(s) on the document(s) in the presence of Consular Officer – not applicable in case of commercial documents as these need first to be notarized by the State Department of the State applicant live in.

 

8.      Fee:

Ø       Property related         :  $ 20/-

Ø       Non-property related  :  $ 10/-

Ø       Commercial related    :   $ 50/-

9.      Processing Time: if submitted between 9 am & 12.30 pm at the counter, documents are delivered back same day between 3.30 pm to 5 pm.

NOTEAll commercial documents must have attestation from the local State Department before submission to the Consulate.  Role of the Consulate is restricted to the attestation of applicant’s signature with no responsibility of contents

 

F.                 Marriage Certificate   

 

1.      Miscellaneous services form (see 8 below) , duly filled in and signed, with one joint-photo (2 in x 2 in  pasted on it.

2.      Notarised photocopy of first three and last two pages of  both husband and wife’s passports (notarization not required if applying in person at the counter)

3.      Copy of marriage certificate – duly notarized ((notarization not required if applying in person at the counter)

4.      Passports in original to be presented to the Consular Officer for verification and return – if applying in person.

5.      Proof of residence i.e. copy of utility bill or Dr Licence or Lease Deed etc

6.      Fee: $ 20/-

7.      Processing time: same day – if submitted by 12.30 pm in person

                                                     By mail: 3 to 4 weeks.

G. Birth Certificate:  

 

1.      Miscellaneous services form (see 8 below), duly filled in and signed with a recent photograph

2.      Notarised photocopy of first three and last three pages of the current Passport  (notarization not required if applying in person).

3.      VISA status:   Copy  of Pmt. Res. Card or work authorization card or copy of F1/ H1/B1 etc 

4.      Proof of residence i.e. notarized copy of utility bill or Dr Licence or Lease Deed etc in the name of the applicant (notarization not required if applying in person).

5.      Passport(if applying in person) – Consular officer  may demand passport for verification .

6.      Fee: $ 20/-

7.      Processing time:  submitted on the counter by 12.30 pm : same day

                                    By mail: 15 to 20 working days from the date of receipt in Consulate (not from the date of mailing by applicant)

H.  Life Certificate    (service not available via mail)  

 

1.      Miscellaneous services form( see 8 below), duly filled in and signed, a photo pasted thereon

2.      Photocopy of passport (first two and last two page)

3.      Proof of residence i.e. copy of utility bill or Dr Licence or Lease Deed etc

4.      Passport in original to be presented to the Consulate Officer, if asked for.

6.      A photograph to be provided for affixing on the Life Certificate

NOTE: Applications for this service are not accepted by mail.  In exceptional cases,  where the  applicant  is medically unfit to present himself/herself before the Consular Officer, service can be rendered by mail if application is accompanied by Doctor’s certificate stating that the applicant appeared before him (Doctor) on the date of issue of the certificate. Such a certificate should reach the Consulate within a week from the date of issue.  Processing time in such cases will be 15 to 20 working days.  No enquiries are entertained before expiration of the said period.

 

 

I.    Death related documents:  

 

        a.   Transportation of Body

 

1.      Miscellaneous services form (see11 below), duly filled in and signed by the person intimating the death.

2.      Intimation of death – on plain paper duly signed by the person intimating the death, alongwith a copy his/her (informant) passport (first two and last two pages only), copy of his Driving Licence or State I.D. or utility bill or any other document showing complete address.

3.      Indian passport (of the deceased) – which will be cancelled and returned.

4.      Death Certificate (original and a copy) issued by the Hospital – for attestation

5.      Embalming Certificate (original and a copy) for attestation

6.      No Communicable Disease Certificate (original and a copy) for attestation

7.      Fees:  No fee   

8.      Processing time: while u-wait if presented at the Counter. By mail it may take time depending upon rush of work.

9.      Mailing Fee (if the documents are to be mailed back ): Express Mail: $ 20/-; Priority mail: $10/-  OR adequately stamped self addressed envelope for return of documents.  No tracking numbers are furnished for Priority Mail and SASE

b.   Transportation of Ashes to India 

 

1.       Misc Services Form, duly filled in and signed by the person intimating the death.

2.       Intimation on plain paper duly signed by the person intimating the transportation of ashes of the deceased, alongwith a copy his/her (informant) passport (first two and last two pages only), copy of his Driving Licence or State I.D. or utility bill or any other document showing complete address.

3.       Indian Passport of the deceased for cancellation.  (not required if holding foreign passport)

4.       PIO Card/OCI document for cancellation – if held by the deceased.

5.       Death Certificate (original and a copy) for attestation

6.       Cremation Certificate (original and photocopy) for attestation

7.       Fee: Gratis

NOTE:  IT IS DESIRABLE TO GET THE ORIGINAL CERTIFICATES ATTESTED BY THE CONSULATE TO ENSURE CLEARANCE THROUGH CUSTOMS AND IMMIGRATION CHECK POINTS IN INDIA.

 

   c.     In cases where neither body nor ashes being transported to India.

 

1.       Misc Services form, duly filled and signed by the person intimating death.

2.       Written intimation of death duly signed by the informant, clearly indicating informant’s relationship with the deceased.

3.       Passport of the deceased – it is cancelled and immediately returned to the informant.

4.       Death Certificate in original.

5.       Copy of the above.

6.       Cremation/burial Certificate in original.

7.       Copy of the above.

8.       Fee: Gratis (if death registration certificate is required.)

K.   Police Clearance Certificate:   The process is completed in two stages.

       Stage 1:

 

1.       Miscellaneous Services (see 6 below) form, duly filled in and signed (no column be left blank).

2.       Notarised copy of passport (first three & last two pages) – DO NOT SEND  PASSPORT AT THIS STAGE.

3.       Proof of residence i.e. copy of utility bill or Dr Licence or Lease Deed etc

4.       Referal Fee: $ 20/-  (for seeking clearance from Passport Issuing Authority) – only in cases where passport has been issued from a place other than Chicago. This fee is not charged if the Passport has been issued from Chicago more than 5 years ago. However if the passport issued from Chicago is less than 5 years old, the referral fee is required.

5.        Mailing Fee: Express Mail: $ 20/-;  Priority mail: $10/- OR adequately stamped self addressed envelope for return of documents.  No tracking numbers are furnished for Priority Mail and SASE

       Stage 2

 

  1. Misc Services form (see 6 below), duly filled in and signed, clearly indicating the reason of seeking PCC i.e. for immigration to……  etc etc.
  2. Passport in original – for making an observation of having issued PCC.
  3. Fee : $ 20/- for making observation at 2 above.
  4. Mailing Fee: Express Mail: $ 20/- (plus $ 2/- for each addl ppt); Priority mail: $10/- (plus $2/- for each addl ppt) 
  5. Processing time: Stage 1: 50 days from the date of receipt of application in the Consulate Stage 2:  In person: same day

                      By mail:  2 to 3 weeks from the date of receipt of Passport in the Consulate

NOTE While mailing application for first stage, envelope containing application should be clearly marked in bold letters: “Application for PCC”.  It will help if e-mail address is indicated on the Misc Services form so as to establish contact, if requiredAs soon as clearance is received, applicant is intimated for commencing the second stage.     PCC is issued within 10 w/days from the date of receipt of Passport in the Consulate.  Envelope containing documents for the 2nd stage should be clearly marked in bold letters : “FOR  PCC”  No queries entertained before the expiration of processing period indicated above.

 

 

L.   No Objection/Obligation to Return to India (NORI):  The process is completed in two stages.

 

       Stage 1:

 

1.       Misc Services form  (see 8 below), duly filled in and signed by the applicant.  No column be left blank

2.       Duly filled in Affidavit  - 4 copies (could be downloaded alongwith form – see 8 below)

3.       NORI form :  see 8 bwlow

4.       Proof (any two) of residence i.e. copy of utility bill/Dr Licence/Lease Deed, state ID etc

5.       Notarised copy of passport (first three and last 2 pages) – do not send passport.

6.       Fee: $ 60/- (for attestation of 3 copies each of 2 and 3 above @ $ 10/- per attestation)

NOTE:  It is the sole responsibility of the applicant to obtain “No Objection Certificate” directly  from all the three agencies i.e. concerned Passport Office, concerned State Government and Ministry of Human Resource Development.  This Office is not responsible for any follow up action with either of the agencies stated above.  On receipt of said documents please proceed to Stage 2 and submit requisite documents.

 

       Stage 2

2.       Originals of “No Objections” obtained from all the three agencies

  5. A copy of Bio-data with address, telephone number, e-mail addres/fax number

3.       Statement of the applicant, duly signed,  giving reason for seeking EC

4.       Old Passport or documentary evidence, acceptable to the Consular Officer, to establish Indian identity of the applicant beyond doubt.

5.       Photocopy of the passport (first two and last two pages) – if available.

6.       Copy of visa status, if applicable.

7.       Proof of residence i.e. copy of utility bill or Dr Licence or  Lease Deed etc. This required to ensure that the applicant is a resident of the area covered by the Consular jurisdiction of the Consulate.

8.       Photographs : 6 coloured recent photographs  of 3.5 cm x 3.5 cm with full frontal view against white background only.
